Deadline Extended for Use of Old Federal CCFs

Alere Toxicology previously notified our clients of our newly designed Federal Custody and Control Form (CCF) that is in accordance with revisions to the Mandatory Guidelines effective October 1, 2010. The use of the previous version of the Federal CCF was permitted through September 30, 2011 to allow for depletion of existing supplies. HHS and DOT have extended this deadline to November 30, 2011, so our clients may use old forms until then.

Beginning December 1, 2011, the use of the old Federal CCF must be handled as a correctable discrepancy. To avoid a delay in the processing of specimens collected on old forms, please ensure you have the proper materials. Since April, Alere has supplied the revised forms in response to new orders. If you have not ordered and received Federal CCFs within the last five months, we recommend placing an order prior to November 10 to ensure delivery before the new deadline.
